Job Description

Key Responsibilities

Curative Care Support (Facility-Linked)

  • Case Identification & Triage: Identify symptomatic cases (e.g., persistent cough, fever, skin infections) in the community and facilitate immediate entry into the clinical system.
  • Direct Curative Support: Under clinical supervision, provide community-level treatment for uncomplicated malaria, pneumonia, and diarrhea (iCCM protocols).
  • Treatment Adherence: Monitor patients on long-term curative regimens (e.g., TB-DOTS or HIV/ART) to ensure compliance and reduce default rates.
  • Post-Facility Follow-up: Visit patients recently discharged from the site to monitor recovery and ensure they understand their prescription instructions.

Preventive & Promotive Care

  • Targeted Screening: Conduct blood pressure, blood glucose, and mid-upper arm circumference (MUAC) screenings to identify at-risk individuals before acute symptoms develop- according to availability of resources and program.
  • Maternal & Newborn Care: Map all pregnant women in the catchment area; ensure they attend at least four Antenatal Care (ANC) visits at the site.
  • Expanded Immunization (EPI): Maintain a community "defaulter-tracing" system to ensure no child misses a scheduled vaccination.
  • NCD Prevention: Lead community groups focused on lifestyle modifications for Diabetes and Hypertension management.

Referral & System Navigation

  • Escalation Mapping: Differentiate between cases treatable at the community level and those requiring the specialized diagnostics of the site.
  • Emergency Dispatch: Recognize "Danger Signs" in children and adults, arranging for immediate transport and pre-notifying the facility.
  • Feedback Loop: Ensure that the results of a referral are communicated back to the family and recorded in the community health log.

Operational Standards

  • Integrated Care: The CHW will use standardized "Sick Child" recording forms.
  • Quality Assurance: Participate in monthly peer-review meetings at the site to discuss referral outcomes.
  • Data Integrity: Update the Facility-Community Linkage Register to track the "Success Rate" of preventive interventions.
