Job Description

TASKS & ACCOUNTABILITIES

  • Undergo CITI training for biomedical researchers, clinical research associates and good clinical practice (GCP).
  • Travel to outreach and SCC as necessary to support patient referrals and follow-up care.
  • Referral Management:
  • Ensure referral forms are accurately developed and readily available in places of need once key locations are identified.
  • Identify & discuss potential patients with consultants that can be referred to SCC or outreach center from the hub for access to care close to patients’ homes as per patients home address.
  • Collaborate with consultants to accurately complete referral forms for patients, with all necessary patient information.
  • Submit referral forms for review and approval by the consultant.
  • Share referral form to the referral centre electronically, prior to the move of the patient –same day of referral.
  • Patient Support & Communication: 
  • Act as a point of contact for patients requiring referrals, ensuring they understand the referral process and what to expect. 
  • Address patients questions or concerns they may have regarding referral process. 
  • Ensure that patients are kept informed and up to date on the status of their referrals. 
  • Collaboration with Healthcare teams
  • Coordinate closely with consultants and healthcare professionals (at Hub & SCC) to ensure referrals are processed efficiently and effectively 
  • Maintain strong working relationships with staff at outreach and SCC.  Identify and address socio-economic problems. 
  • Assist the team with the collection of specific missing data for individual referred patients as required. 
  • Administrative Tasks
  • Maintain accurate records of all patient referrals and communications · Ensure all referral paperwork is completed on time and in accordance with organizational guidelines. 
  • Assist with reporting and tracking of referral outcomes as required by the team. 
  • Quality Assurance: 
  • Monitor referral processes to ensure they meet quality and safety standards.
  • Identify opportunities for improvement in the referral process and contribute to the development of best practices.
