Job Description

• • Conduct outreach activities in the community to identify and refer people living with HIV (PLHIV) to the ART center
  • Establish and maintain strong linkages with community leaders, organizations, and health facilities for referrals.
  • Provide adherence counseling to PLHIV and their families during home visits
  • Conduct home visits and follow-up calls to track missed appointments and lost-to-follow-up (LTFU) patients.
  • Educate communities on HIV prevention, testing, and treatment services.
  • Address myths, misconceptions, and stigma around HIV/AIDS.
  • Submit daily/weekly activity reports to the field supervisor.
