Job Description
A. MANAGEMENT OF RESOURCES
- Review staffing in each shift and assign accordingly.
- Ensure ward inventory checking and record in checklist after physical verification in every shift.
- Ensure process of material resources (supplies, medication, equipment and supportive service).
- Make sure bed to bed patients' handover and handover documentation in HMIS.
- Ensure no duty is exchanged without filling duty exchange form dully signed by AHN/HN/DM.
- Initiate report for absenteeism, uninformed leaves and discipline and share with areas AHN/HN.
- Demonstrate and ensure professional appearance (hygiene, neat & clean uniform/kit & shoes) and all the staff working in assigned areas and comply with tea/meal break timings as agreed mutually.
- Visit all areas, meet patients for service evaluation and medication and other supplies availability.
- Resolve patient/family's problems by involving multidisciplinary teams and ensure documentation.
- Ensure cleanliness by in person visit of the ward and rest rooms, staff lounges and coordinate with the Housekeeping staff/supervisor if not found compliant particularly during evening and night shifts.
- Ensure safe working environment for staff, patients and patients visitors by ensuring compliance with procedures, rules and regulations in collaboration with security.
- Ensure compliance with Hand Hygiene & all infection-control practices to protect employees, patients, attendant & environment (terminal cleaning of beds & equipment after patients' discharge, medication administration, cannulation, sampling, cleaning of trays after use, keeping items for one patient, storage, and control of chemicals.
- Guide new staff regarding work routine in accordance with hospital policies, protocols and SOPS Identify & share training needs of the staff experienced and new staff to DM/CNI/HN/AHN.
- Provide coverage for the Ward in charge/Team Lead during breaks/ and shortage.
- Ensure entire shift nursing care activities across the hospital/in the assigned Unit including hygiene care of patients and its documentation.
- Ensure accurate Monitoring, documentation and reporting of vital signs (TPR, BP, 02 Sat & CTG etc.), entries in HMIS
- Ensure timely collection (Lab staff & nursing) their transportation to the Lab and reports are followed by medical & nursing staff as applicable.
- Ensure accurate intake and output record at the end of each shift (cumulative 24hrs) including positive & negative balance in patients' records and report to doctor if balance is not as per plan of patient care management.
- Ensure medical and nursing team is working in collaboration for various medical & surgical procedures.
- Ensure medications process (prescription, pharmacy dispensing and administration) as per doctor's order and administration is documented/written in notes. Near miss reporting instantly if any error found during dispensing verification process and medication got corrected as a corrective action.
- Make sure independent double check is carried out at patient bed side before administration of high alert medication, transfusion Blood & blood products and documented in patient record.
- Ensure patients pre-operative/invasive procedures preparation is checked before transfer (consents, blood arrangement, NPO, site preparation, bath and documents, including pre-op medication as per pre-operative physician orders and fresh vital signs including height & weight.
- Make sure that patient's admission & discharge procedure is carried out by assigned staff and records are maintained.
B. QUALITY ASSURANCE
- Participate in Nursing audits for KPI compliance monitoring and Documentation. (initially nursing assessment, NCPs' (Fall prevention, cluster NCP of general patient and Obs/Gynae), medication administration).
- Actively Participate in PHC audit preparation.
- Ensure Crash Carts checked, Locked and checklists are filled in each shift.
- Make sure timely reporting of incidences with prompt investigation (nature of event, corrective action taken, documentation in patient recorded whenever needed and staff statement and corrective action needed to avoid reoccurrences.
Perform any other task assigned by the immediate Supervisor and/or Head of Department that is commensurate wit your knowledge and skills.
