Job Description

GENERAL RESPONSIBILITIES:

  1. Supervise to control the services, responsible for staffing, duty roster management, and to ensure proper functions of Clinical Pharmacy, Central Pharmacy (In Patient, Sterile & nonsterile compounding Pharmacy services or entire pharmaceuticals care services), according to the scope of healthcare facility/Campus.
  2. To monitor the shift wise handing over & taking over charge of assigned pharmaceuticals care locations ensure implementing the SOPs & protocols.
  3. To monitor evidence based dispensing and timely distribution of medications from the assigned Pharmacy locations (Central Pharmacy, Satellite Pharmacy, In-Patient Pharmacy & Clinical Pharmacy services.
  4. Monitor to proceed the non-formulary items requests, to review and guide the Clinical Pharmacy team for available therapeutic alternative or alternative class of drugs or to procced arranging LPs following the NFI policy.
  5. Ensure, updating the Inventory stock level, Departmental requests for timely replenishment of supplies from the Warehouse or Bulk stores.
  6. Monitor to ensure the storage, on specified temperature, to follow the SOPs (room temperature, refrigerate, thermolabile, photosensitive, high alert, LASA etc.), issuances by FEFO/FIFO, & timely dispatches of supplies at Nursing unit or at satellite Pharmacies.
  7. Ensure expiry management, timely consumption or to follow up with Warehouse, informing for short expiry, slow moving, dead stocks return or replacement, & also to submit its periodic reports to Pharmacy management.
  8. ADRs, ADEs, management, its monitoring, documentations & reporting to the Pharmacy Management, QIPs & DRAP.
  9. Departmental daily activities, updates, & KPIs monitoring, to update its regular reports to the Pharmacy management including Pharmacy Interventions, Prescription Errors, Dispensing errors, & Near Miss errors data.
  10. Submit monthly Pharmaceuticals, Med-surg consumption, & stock out reports with suggested action & strategies for presenting to C-P&TC & N-P&TC  
  11. Follow-up with QIPs to respond on incidents reporting, related to the Pharmacy services.
  12. Help in management & monitor training of interns and share their training assessment report to Pharmacy Management.  
  13. Monitor & to submit periodic surprise audit reports of Inventory & Pharmacy services locations.
  14. Collect, compile all data and share timely with Pharmacy management
  15. Ensure take care of Pharmacy services Instruments, equipments & its timely PPMs, validation & required services.

ESSENTIAL RESPONSIBILITIES IN A CLINICAL PHARMACY SERVICES:

  1. To Ensure implementation, of clinical Pharmacy SOPs, activities, emphasizing on the therapeutic drug monitoring, clinical pharmaceuticals care, Formulary management, pharmaco-economics and patient safety with desired treatment outcomes through detailed reporting systems.
  2. Assists the Pharmacy Management in developing operational systems and procedures for the Pharmacy Department’s Clinical Division, in accordance with the local regulatory compliance, guidelines of the Joint Commission on the Accreditation of Healthcare Organizations, HCC, & DRAP.
  3.  Coordinates the activities associated with the provision of safe, effective and most economical pharmacotherapy for patients' treatment, especially in the Pharmacy's monitoring programs.
  4. Collaborates with physicians in establishing drug criteria for the safe, economical and appropriate use of medications.
  5. Participates in multidisciplinary team rounds and provides pharmacotherapy recommendations during the discussions.
  6.  Develops disease/drug therapy management programs to ensure safe, appropriate and cost-effective use of medications.
  7.  Manages non-formulary requests for drugs, and to participate in the P&T Committee activities.
  8. Performs initial assessments of drugs having pre-established criteria to be monitored, and monitors these drugs for follow-ups.
  9. Participates in pharmacoeconomic evaluations and in treatment outcomes research.
  10. Retrieves, analyzes, evaluates, and interprets the scientific literature, & population specific drug information for patient and health professionals.
  11. Assists the Pharmacy management, review, tracking and compilation of data regarding adverse drug events (i.e., medication incidents and adverse drug reactions);
  12.  Issues statements on the clinical policies and procedures of the Pharmacy Department.
  13. Assists in the selection of drug products and in developing the drugs protocols.
  14. Participates in the generation of new knowledge relevant to the practice of pharmacotherapy, clinical pharmacy and medicine; Educates and provide drug information to health care professionals, students, patients, and the public regarding rational therapy.
  15. Data collection, compilation, analysis, Interprets and applies pharmacokinetic drug data, and recommends dosing regimens based on patient's medical history and disease state, ensuring optimal patient care.
  16. Develops communication systems to respond to problems or emergency requests from patient care units. 

ESSENTIAL RESPONSIBILITIES IN THE INPATIENT PHARMACY:

  1. Monitor proper dispensing, storage, requisitions, receiving, Distribution and reporting.
  2. Monitor unit dose dispensing, controlled/Narcotics medications dispending & documentation.
  3. Monitor the handling of patient’s own medications,
  4. Monitor the operations and documentation of sterile, non-sterile compounding
  5. Monitor the operations of clean room operations, Chemo & IV admixtures' preparations, documentation & its timely distribution
  6. Monitor the operations and documentations of sub stores

ESSENTIAL RESPONSIBILITIES IN THE OUTPATIENT PHARMACY:

  1. Monitor proper dispensing, Patients counselling, storage, requisitions, receiving, reporting.
  2. Monitor and collect the data of clinical interventions, Drug information

ESSENTIAL RESPONSIBILITIES IN THE ER PHARMACY:

  1. Monitor proper dispensing, counselling, storage, requisitions, receiving, reporting.
  2. Monitor the Pharmacist round to the patient bed side in ER Monitor the operations and documentations of ER Pharmacy.
