Job Description
The Head Nurse (HN) is responsible for the planning (workforce & ma material), organization, implementation and evaluation of nursing care activities as per IHHN's policies. Head Nurse assists Manager/Deputy Manager in managing assigned areas, best possible patients' care & clinical operation within the context of professional nursing practices including infection control & patient safety as an approach to continual quality Improvement. Take active lead in PHC audits/High profile round preparation and accompany as required.
TASKS & ACCOUNTABILITIES
A. PATIENT CARE MANAGEMENT
- Make sure in the beginning of that schedule staff of the shift arrived, if some schedule staff didn't turn up, review the available staff & their workload and re-allocate (staff & additional tasks) from low acuity areas to high acuity areas for the effective utilization of present staff and if hard to manage patient activities, ask for extension of duty of previous staff during rush hours and keep last option for double duty. No night staff to would do double duty or stay late for any meeting and concern must be handed over to morning shift to carry forward.
- Make sure staff allocation of all areas completed in log book as well as on areas white boards in all shifts, in accordance with patients' acuity, including special assignment & staff break timing to avoid inconvenience.
- Carry out quick round, to evaluate overall patient care, resources required, their availability and pay swift attention to urgent one.
- Coordinate with previous shift supervisor to inquire last 24hrs report (concerns/issues/event, related to staff/, patients & attendant and handle on priority bases.
- Conduct daily 10 minutes morning briefing with staff to discuss concern about (staffing, patient care, incidents, complains, appreciation or any other concern).
- Ensure staff compliance with duty hours (uniforms/attire, attendance & punctuality, hospital cards).
- Make sure Healthcare Worker behave professional with (patient/attendant and co-workers with the reflection of Indus Code of Ethics.
- Ensure Ward/s' are ready for physicians' round including patients'/beds, and surrounding.
- Make sure bed to bed patients' handover is in practice in all shifts and documentation in patient record.
- Join handover on regular bases in duty shift or designate to next level if you are busy in another task.
- Make sure designated Ward/area inventory is physically verified against checklist & marked in each shift.
- Conduct random checks of inventory/crash cart/stock to make sure compliance, share areas of improvement and ensure corrective preventive actions.
- Ensure assigned staff is aware of patients' care needs hygiene care & change of clean hospital dress, bedding
- Make sure physician's round accompanied by area in charge & patients assigned staff.
- Coordinate with consultants/medical officers to discuss patient care, personnel issues & resolve
- Ensure availability of consumable supplies and record of consumption
- Make sure staff enter, follow supplies & consume used supplies in HMIS.
- Hold weekly meeting with senior nursing team (NSSs', & TLs) to review staffing & nursing service out comes.
- Liaise with CNI on daily/weekly bases to discuss in-service education needs, training & development activities.
- Keep the Nursing Management informed of any special needs of the ward under supervision.
- Conduct random round with staff to see their allocated patient care & overall management.
B. ENSURING PATIENTS MONITORING, ASSESSMENT, CARE PLANS AND DOCUMENTATION IN PATIENTS RECORD
- Make sure completion & review of patients' initial assessment, prioritizing of care needs & completion of NCPs" (Fall prevention, Cluster NCP and Cluster Oby/Gynae NCP).
- Make sure staff monitor fresh vital signs & height & weight of new admissions and documentation in patients' record to have BSA calculated by HMIS.
- Ensure provision of hygiene care as per patients' individual needs (Activity Daily Living) of bed bond/ activity restriction due to disease requirement (AM & PM care) & documentation.
- Ensure each patient's vital signs, height & weight checked and documented as per monitoring order/policy.
- Ensure exact calculation of intake and output, drainage and documentation in patient record.
- Make sure prescribed Investigation of the patient (Labs, Urine and stool specimens) are collected, labelled & dispatched to the Lab for early results.
- Ensure Care of I/V line/ cannula, CVCs' lines and drains and tubes is taken as per IHHN guideline.
- Ensure dressing of wounds is carried out following IC guideline.
- Extend help in direct patient care, at times of staff shortage and in emergency situations.
C. MEDICATION MANAGEMENT
- Make sure staff is aware of medication management flow process (prescription, Identification of drugs, dispensing, physical verification of dispensing & acknowledgement in HMIS, verification against rights of Drugs safety prior to preparation, labeling of prepared drug, administration after patient & drug safety identification and documentation in patient's HMIS record.
- Make sure staff is aware of Not available (NA) medication process, its requisition.
- by duty doctor and documentation in patient record about LP form & follow up with pharmacy.
- Ensure timely dispensing and administration of medication in accordance with drug/patient safety rights & its documentation in patients' HMIS record or in nurses' event if module is not available in HMIS.
- Ensure practice compliance with independent double check of High alert /LASA medications, blood & blood products transfusion prior to preparation and prior to administration at patient site or bed side.
- Initiate incident report if drug is not as per order, take pictorial evidence and resend drug to pharmacy for change, so patient will get drug as early as possible.
- Make sure renewal of medication for patients during IPD admission on daily bases.
D. MONITORING OF INVENTORY AND MAINTENANCE OF EQUIPMENT
- Make sure designated Ward/area inventory is physically verified against checklist & marked in each shift.
- Ensure staff training from Biomedical about equipment operation, cleaning and functionality.
- Make sure that missing inventory is reported to accountable & HOD decide official reporting.
- Ensure cleaning and disinfection of equipment as per IC guideline and their safety.
- Ensure out of order/broken equipment reported to biomedical for repair, label out of order & followed.
E. MONITORING OF STAFF PERFORMANCE AND AREAS OF IMPROVEMENT
- Evaluate staff teaching needs & organize Ward Teaching Program accordingly
- Participate in clinical teaching and staff development programs.
- Counsel and guide all nursing staff as per their individual needs.
F. QUALITY ASSURANCE PERSPECTIVE
- Ensure compliance with the IHHN policies and procedures in all shifts.
- Complete tasks assigned by the Manager/ Deputy Manager (Audits, Indicators monitoring & complaints /incidents investigation etc.
- Report and encourage staff to report incidents after consulting with shift supervisors/senior if necessary.
- Facilitate /escort special visitors on the hospital round.
- Participate in department, Nursing Division and QMD CQI KPL
- Take part in 24hrs, monthly and quarterly reports
- Prepare assigned areas for audits (Nursing, QIPS, IHHN internal/external, & PHC etc.).
- Ability to initiate incident /sentinel events, investigate including and timely completion in collaboration with QAD.
G. HUMAN RESOURCE MANAGEMENT
- Make sure that new joiners orientation objective is discussed with staff to complete within probationary period.
- Ensure smooth running of department orientation of new joiner, their clinical placement with new joiner orientation checklist, discuss monthly progress of objective with individual staff, indicating areas of improvement and keep record.
- Ensure finalization of timely duty roster a week earlier of schedule month, enter in HMIS, display on the notice board for the early view of staff and keep a signed copy for each category for daily review.
- Ensure staffing budget as per agreed nurse patient ratio with HR, staff hiring collaboration with HR.
- Ensure timely probationary confirmation with formal meeting and constructive feedback.
- Review biannual individual in person objective of staff to discuss areas of improvement with anecdotal record
- to use in summative & formative performance evidences to avoid disagreement.
- Review annual performance with each staff in person against objective, share strengths & weaknesses.
- Communicate timely resignation/absenteeism/nonprofessional behavior of staff to HRD.
- Ensure personnel growth, renewal of competencies & certifications.
