Job Description
- Conducting door-to-door household visits
- Organizing and facilitating community awareness sessions and school-based sessions
- Screening, referral, and follow-up of ANC, PNC, and EPI due and missed cases
- Referring eligible beneficiaries to Mobile Health Teams and Facilitation Centers (FCs)
- Coordinating with LHWs, teachers, Union Council Chairmen, religious leaders, and community elders
- Maintaining daily activity logs, beneficiary lists, and reports
- Supporting monitoring, evaluation, and donor reporting
- Identification of malnutrition and referral pathways
- Stakeholder identification and engagement
- Development of taluka-specific action plans
- Social Mobilization component, the objective is to mobilize due-follow up cases (Red & Yellow category) for timely assistance and to address compliance issues and facilitate them for issues like CNIC, B-Form and other issues.
- Due-follow up compliances of yellow and red categories of beneficiaries, to resolve the documentation issue
