Job Description
Job Summary:
- Acts as the Strategic Bridge between program leadership and field implementation. The Community Engagement Manager is responsible for developing, coordinating, and maintaining strong relationships between primary care clinical sites, patients, community organizations, healthcare partners, and other local stakeholders.
- The role focuses on increasing awareness and utilization of primary care services, improving patient access and engagement, supporting health education and prevention initiatives, and ensuring that clinical sites effectively understand and respond to the needs of the communities they serve.
- The Community Engagement Manager will work closely with clinical, operational, outreach, and administrative teams to develop community-centered strategies that improve health outcomes and strengthen trust in primary care services.
- Responsible for turning high-level strategy into actionable field policy and ensuring 'One-Team' unified architecture.
