Job Description
Coordinates the workup of transplant recipients and donors, pre and post-transplant patient care for organ recipients and donors. Performs a variety of tasks in clinical settings in collaboration with medical and nursing staff and accountable for the assessment, planning, implementation and evaluation of care to specific patient population.
- Coordinates kidney transplantation with patient, donors, families, medical staff, organ transplant authority and administrative staff.
- Maintains and regularly reviews status of patients on the waiting list for transplantation and ensure all diagnostic investigations and results are up to date
- Organizes and participates in multi-disciplinary team conferences, rounds or meetings related to care of transplant patient.
- Documents all relevant information correctly, appropriately and in a timely manner.
- Participates in all mandatory departmental activities i.e. Staff Meeting, Core Lectures, Research Forum, etc. and serves on departmental and hospital committees, as required
- Actively contributes to the Division’s and/or the Department’s documentation of patient care processes and outcomes
- Upholds the organizational values at all times.
- Comply and follows with all hospital related policies and procedures.
